315 West 88th St, between West End and Riverside. You're right next to Riverside Park, which is lovely; very quiet street, mostly townhouses, Brooklyn-esque. Plenty of good cafes/workspaces two to three blocks away -- and of course Central Park is not far. There's also Lincoln Center about 10 min on the subway or a nice 15-20 min stroll (great movie theater there, as well). Westsider Used & Rare Books nearby, W 81st I believe, one of the very best bookstores in New York. And some great neighborhood restaurants, such as the longstanding Gennaro, Amélie, (these are both 5 minutes away), Le Vin Coeur, etc. New Cottage for Chinese, is terrific. Penn Station is a 15 minute subway ride on the red line 1-2-3. There is an entry two blocks away, and bus stop in the same location. Very convenient.
It's a lovely sunny place, south-facing, big windows. Beautiful old townhouse, really classic. Hardwood floors, built-in bookcase, room for a queen bed, nice roof which is great in the warmer months. Big coat/storage closet with another storage closet above it, actually quite spacious. No elevator, as it's an old building, so it's three (relatively small) flights of stairs up. Kitchen is small, but appliances new. Note: no laundry machines. Plenty of laundry places nearby, of course. I use one right around the corner, on W 87th. Utilities are all included in rent except electricity: I usually pay about $40 / month. I am also willing to leave FiOS Wifi router; we can transfer account.
The supers are fantastic, and the management accomodating. I've had no issues there. Quiet congenial neighbors. I wouldn't be moving out if it weren't for a PhD opportunity I need to pursue.

You will need to provide recent taxes, there'll be a lease agreement of course, income guarantee or guarantor, and security deposit = the monthly rent. Not a co-op process or anything like that. I was in 3-4 days after signing the lease.
Cats are allowed, but no dogs. Service animals are of course an exception.
